OPTION 1: I am only interest in attending/Participating in PTO Events at this time (no volunteer/planning duties) - Please add me to your newsletter
OPTION 2: I’d like to be notified of upcoming events, meetings and volunteer/participate for opportunities as they arise on a noncommittal basis (Annual Yearly Membership: $5) + $5.00
OPTION 3: I’d like to be a fully participating member who regularly attends meetings & is actively involved in planning & executing events (Annual Yearly Membership: $25 - PTO tee-shirt included - please indicate your uni-sex size below: small_med._Large _XL _XXL _or XXXL) + $25.00

I will send my cash payment in to school (please include a paper copy of the form and the money in an envelope clearly marked PTO)
